Hemoptysis is the expectoration of blood or blood-stained mucus from the lower respiratory tract. It is a symptom that can cause significant distress and generally warrants careful medical evaluation.
The presentation can range from scant, blood-streaked sputum to substantial hemorrhage, which can represent a respiratory emergency.
Understanding the Source
When evaluating expectorated blood, physicians consider whether the source is the lower respiratory tract (true hemoptysis) or elsewhere:
- Pseudohemoptysis: Blood originating from the upper respiratory tract (e.g., the nasal passages or pharynx) or the oral cavity.
- Hematemesis: Vomited blood originating from the gastrointestinal tract, which may appear darker or be mixed with food particles.
The lungs have a dual vascular supply: the pulmonary circulation (low pressure, involved in gas exchange) and the bronchial circulation (high pressure, supplying the airway tissues). Many cases of significant hemoptysis, particularly those associated with chronic airway inflammation or architectural distortion, originate from the high-pressure bronchial circulation.
Potential Causes of Hemoptysis
The etiology of hemoptysis varies based on factors such as geography, patient age, and clinical history.
Infectious and Inflammatory Causes
- Pulmonary Tuberculosis: A frequent cause of hemoptysis in endemic regions.
- Bronchiectasis: Abnormal widening of the airways, often a consequence of prior infections (such as Post-Tuberculosis Lung Disease). Chronic inflammation can lead to the proliferation of fragile bronchial blood vessels that may bleed.
- Lower Respiratory Tract Infections: Acute bronchitis or pneumonia can sometimes present with blood-streaked sputum.
- Fungal Infections: Such as an aspergilloma developing within a preexisting lung cavity.
Neoplastic Causes
- Lung Cancer: Hemoptysis can be a presenting symptom of bronchogenic carcinoma, particularly in individuals with a history of tobacco use or relevant occupational exposures.
Cardiovascular and Other Causes
- Pulmonary Embolism (PE): A thrombus in the pulmonary circulation can cause lung infarction and hemoptysis, often accompanied by breathlessness or chest pain.
- Vasculitic Disorders: Autoimmune conditions that cause inflammation of pulmonary blood vessels.
- Medications: Anticoagulant therapy can increase the risk of bleeding from minor respiratory pathologies.
Clinical Evaluation
The evaluation of hemoptysis aims to identify the source of bleeding, determine the underlying etiology, and assess the severity.
Clinical Assessment
A physician will take a history regarding the estimated volume and duration of bleeding, associated symptoms (such as fever, weight loss, or chronic cough), and risk factors like smoking or prior lung disease.
Investigations
- Chest Radiograph (X-ray): Often the initial imaging modality to assess for gross abnormalities, though it may not detect small lesions or early bronchiectasis.
- Computed Tomography (CT): CT, particularly CT angiography, is frequently utilized to provide detailed imaging of the lung parenchyma, airways, and vasculature to localize the source and potential cause of bleeding.
- Laboratory Tests: May include sputum analysis, complete blood counts, and coagulation profiles.
- Bronchoscopy: Endoscopic visualization of the airways may be considered to directly identify the bleeding site or obtain samples from suspicious lesions.
Management Principles and Severity
The management of hemoptysis is guided by both the underlying cause and the clinical severity.
Importantly, the severity of hemoptysis—and the potential danger it poses—is not defined solely by the exact volume of expectorated blood. While larger volumes are generally more concerning, clinical severity is determined by the rate of bleeding, the patient's underlying respiratory reserve, their ability to protect their airway, and any resulting hemodynamic instability or airway compromise [2].
- Mild Hemoptysis: Often addressed by diagnosing and treating the root cause (e.g., treating a respiratory infection).
- Severe or Life-Threatening Hemoptysis: This is a medical emergency. The primary immediate risk is often asphyxiation (airway obstruction by blood) rather than hemorrhagic shock. Initial management focuses on securing the airway and ensuring adequate oxygenation. Subsequent interventions may include procedures such as Bronchial Artery Embolization (BAE) to occlude the bleeding vessel, or, in specific circumstances, surgical intervention.
When to Seek Evaluation
Because hemoptysis can be a symptom of significant underlying pathology, instances of coughing up blood should be evaluated by a healthcare professional. Urgent medical attention is required if the bleeding is substantial, continuous, or accompanied by symptoms such as severe shortness of breath, chest pain, dizziness, or confusion.
